Developmental Pathways (DP) has an opportunity for a Case Manager (CM) – Intake in our Systems Navigation (SN) Department. The Intake CM is responsible for initial screening and assessment of individuals’ immediate and long-term needs and referring to appropriate internal and external programs and community resources to support assessed needs. They will ensure timely response to internal and external referrals for individuals pursuing programmatic supports. The Intake CM will provide case management, eligibility determination support and waitlist support and updates to individuals waiting for services, as needed and applicable. They are responsible for ongoing screening of individuals needs and referring to appropriate programs and community resources as appropriate. The Intake CM will ensure timely initiation and implementation of initial eligibility assessments and coordination process to access HCBS waiver services or other state-funded programs. Also, will assist with Enrollment duties as needed.
This is a 40-hour per week, Monday through Thursday or Tuesday through Friday, hybrid position, with the benefit of working remotely and in-person coming into our headquarters office in Aurora, CO and/or off-site locations.

Requirements
Bachelor’s Degree; or
Five (5) years of experience in the field of Long-Term Services and Supports; or
(LTSS), which includes Developmental Disabilities; or
Some combination of education and relevant experience appropriate to the requirements of the position
Valid Colorado motor vehicle license and provide proof of motor vehicle insurance
Meet our driving requirements – no suspensions and no more than two (2) tickets in the past three (3) years
Upon request, qualifying need, and approval, driving requirements may be waived in lieu of reliable transportation.
Preferred
One (1) year of experience in developmental disabilities/long-term services and supports
Additional Responsibilities
Serve as subject matter expert for applicable programs and services; serves as resource/point of contact for complex cases
Assess the overall needs of an individual and assist with the exploration of programs, long-term services and supports, and other community resources to meet assessed needs
Assist individuals and families in maintaining eligibility through the completion of required assessments, monitoring, and support planning
Provide care coordination services and interventions by referring, educating, negotiating, and mediating with the individual and external providers/agencies
Complete accurate case documentation in appropriate tracking and databases; prioritize workload and meet all required timelines
Complete required documentation related to monitoring activities; evaluate need and effectiveness of services requested and delivered
Schedule, facilitate, document, and complete follow-up for all required and ad-hoc meetings; disseminate updates to the necessary parties
Ensure the accuracy and timely entry and distribution of required documentation to all necessary parties, including, but not limited to providers, individuals, and family members
Maintain knowledge of state/ federal regulations, policies, and procedures regarding long-term services and supports, Medicaid waiver and state-funded programs, and other community resources
Maintain a professional and ethical manner in all interactions and meet performance quality, customer service, and coordination standards as assigned by department leadership; maintain appropriate boundaries
